Need help submitting your renewal forms?

Completed renewal forms can be submitted in one of several ways to your county, tribal or state agency.

MAIL: Put documents in the return envelope provided, add postage and put it in the mail. If you don’t have the return envelope, see the County and Tribal directory.

ONLINE: Upload the documents online using the Upload Documents link.

IN PERSON OR FAX: Drop off the documents or fax them to the agency listed as the return address on your paperwork. Addresses and fax numbers for agencies are listed in the County and Tribal directory.

PHONE: Give your information verbally over the phone. Contact your county or tribal agency to learn more. Note: Phone renewals may take up to an hour or longer.
