    Integrated Care System Partnerships

    Overview

    The Minnesota Department of Human Services (DHS) is building on current state initiatives to improve performance of primary care and care coordination models. 

    The current initiatives include integrated Medicare and Medicaid special needs plan managed care programs such as Minnesota Senior Health Options (MSHO) and health care homes including the Medicare Advanced Primary Care Practice demonstration. Other initiatives involve provider payment reform through the Integrated Health Partnerships (IHPs), Dual Demonstration Project and Integrated Care System Partnerships (ICSPs). 

    The ICSPs are designed to improve health care access, coordination and outcomes. This is done through payment reform by establishing partnerships between managed care organizations, primary, acute, long-term care and mental health providers. These providers serve seniors and people with disabilities under managed care programs for people enrolled in MSHO, Minnesota Senior Care Plus (MSC+) and Special Needs BasicCare (SNBC). 

    Proposed improvements include development of system-wide performance measures, risk adjustments, provider feedback systems, and risk and gain sharing models specific to the dually eligible (Medicare and Medicaid) population. 

    Integrated Care System Partnerships (ICSPs) timeline 

    • August 30, 2016
      DHS contracted with Bailit Health to evaluate the overall Integrated Care System Partnership (ICSP) initiative. At the end of August 2016, Bailit publishes their final evaluation report (DHS-7427) (PDF) that includes recommendations for the state to make enhancements to the initiative and to better support effective partnerships. Bailit also creates an implementation plan and the Dual Demonstration team is working to integrate some of the recommendations into the initiative and to determine next steps for the remaining implementation items.
    • December 30, 2015
      State conducts a review of the ICSP initiative since its inception in 2013. The ICSP overview 2015 (PDF) provides a summary of the 48 active ICSP’s best practices, identifies challenges encountered, lists potential strategies for improvement and summarizes lessons learned. MSHO, MSC+ and SNBC measures are made available. Each ICSP must incorporate quality measures and monitor for progress. The complete list of MSHO measures (PDF) and SNBC  measures (PDF) is noted and also highlights the active measures selected by health plans.
    • April 2015
      Health plans submit first descriptive progress report for the 2014 ICSPs.
    • September 30, 2014
      Health plans submit the 2014 ICSP Summary of Performance Measures and Outcomes Report reflecting ICSP progress since implementation on January 1, 2014. (For 2015, there are 54 ICSPs ready for implementation; 36 representing MSHO or MSC+ and 18 representing SNBC.)
    • July 1, 2014
      Health plans submit 19 new ICSP proposals, and DHS approves all of them for implementation in 2015.
    • January 1, 2014
      Implementation for initial 35 ICSPs.
    • October 1, 2013
      DHS approves health plan ICSP contracts with identified partners.
    • July 15, 2013
      Per the 2013 contract, all health plans submit ICSP proposals with identified quality measures. (For 2014, 35 ICSP proposals were received and approved by DHS across eight health plans; 23 representing MSHO or MSC+ and 12 representing SNBC.)

    ICSP historical information

    • February 6, 2013
      The DHS Health Care and Continuing Care Administrations requested public input on best practices for development of ICSPs between managed care organizations and primary, acute, long-term care and mental health providers serving seniors and people with disabilities under managed care programs. The ICSPs are especially designed for people with dual Medicare and Medicaid eligibility, including those enrolled in MSHO, MSC+ and SNBC. DHS received twelve responses to RPI (PDF).
    • January 28, 2013
      DHS held an ICSP Best Practices Symposium to provide information prior to the deadline for written comments and to gather verbal comments on best practices for development of ICSPs.
    • January 22, 2013
      DHS published a request for public input (PDF) and a notice in the State Register about the ICSP development process and specific issues for which public comments were requested.
