Home care nursing
Home care nursing services are more extensive than a skilled nurse visit and can include:
- Professional nursing care based on an assessment of the person's medical needs.
- Ongoing professional nursing observation, monitoring, intervention and evaluation.
Who is eligible for home care nursing?
Home care nursing is available to people who:
- Are eligible for Medical Assistance, an MA waiver, the Alternative Care program or MinnesotaCare Expanded (pregnant women and children)
- Have nursing needs that are:
- Medically necessary.
- Ordered by a physician, advanced practice registered nurse or physician’s assistant.
- Require more continuous care than a single to twice-daily skilled nurse visit.
- Are beyond what a personal care assistant, CFSS worker or home health aid (HHA) can provide.
- Provided according to a written service plan.
People can receive home care nursing services in their home or outside the home when normal life activities take them, including school and work.
Home care nursing services can be provided by:
- A registered nurse (RN) or licensed practical nurse (LPN) employed by a home health agency.
- An independent RN who is enrolled with Minnesota Health Care Programs.
- An independent LPN with a Class A license and who is enrolled with Minnesota Health Care Programs.
- A parent of a minor child, spouse, non-corporate legal guardian or conservator who is a licensed nurse employed by a home care agency and has an approved HCN hardship waiver.
