Minnesota support planning professionals in all fields, including but not limited to:

• Certified assessors
• Case managers
• Relocation services
• Care coordinators
• Support planners
• Person-centered planners
• Managers and supervisors of the above

The support planning professionals learning community holds monthly web-based meetings on the fourth Wednesday from 10 to 11:30 a.m.

The monthly WebEx meetings focus on person-centeredness, with a different topic and guest speakers each session. Each month, we aim to provide the most recent person-centered practices, updates and initiatives from DHS and include policy leads from:

• Disability Services
• Mental Health
• Aging and Adult Services

Note: You must have a unique key to register and receive credit for training. To obtain a unique key, use the Unique Key Request Form.

Register using TrainLink

1. Go to TrainLink.
2. Select Continuing Care under Learning Centers.
3. Select Sign On in the upper right hand corner.
4. Enter your unique key.
5. Select Upcoming Classes/Class Registration.
6. Select All DSD Classes.
7. Search Support Planning Professionals.
8. Locate the session.
9. Select the class.
10. Select Enroll in this class and follow the registration instructions.
