    Tennessen Warning and Talent Release

    Rights, Term of Use, Territory, and Copyright:

    I consent to the use, reproduction, publication, distribution, or assignment by the State of Minnesota and the Minnesota Department of Human Services (collectively “DHS”) of my voice, image, and/or performance in a video, photograph, or audio recording.

    I understand that:

    • I will not be paid for any of the uses of these recordings;
    • I give up all rights to inspect, approve, or control how these recordings will be used by DHS;
    • DHS will not intentionally distort or misrepresent these recordings;
    • My name may be used in connection with all uses of these recordings;
    • The recording remains the property of DHS.

    Notice of Intent to Collect Private Information:

    I authorize DHS to collect private information about me, which may include my name and recordings of my voice, image, and/or performance.

    Access to Private Information:

    This information may be accessed by authorized DHS personnel whose jobs reasonably require access, as well as others authorized under law to access the data. I understand that this information may be used in DHS publications, displays, Web sites, promotional materials, print and/or electronic communications, and/or released to news media organizations. Information used in this way may be accessed and reused by members of the public without notifying or getting permission from me. I understand that I do not have to agree to provide this information to DHS. If I do not agree to provide this information to DHS, I will not appear in the video, photograph, or audio recording.

    Purpose and Intended Use of Private Information:

    Private information about me is provided for the purpose of promoting the public’s understanding of the State’s programs and services and for the purpose of assisting the operations of State government.

    Acknowledgments:

    • I understand that treatment, payment, enrollment, or eligibility for benefits or services from DHS will not be based on this authorization.
    • I agree to indemnify and save and hold the State of Minnesota and DHS, and their representatives, designees, and assigns, harmless from any and all claims or causes of action, including all attorneys’ fees and costs incurred by the State of Minnesota and DHS, arising under performance of this agreement.
    • I HAVE READ THIS DOCUMENT AND I UNDERSTAND ITS CONTENTS.
