Managed care pharmacy identification

In addition to their Minnesota Health Care Programs (MHCP) ID cards, members enrolled in a managed care organization (MCO) also receive ID cards directly from their MCOs. Members are instructed to show both ID cards before receiving health care services.

Verify member eligibility and the MCO enrollment status through the MN–ITS Eligibility (270/271) transaction.

All MHCP-contracted MCO cards contain the following text on the card:

“For appeals, write to the MN Department of Human Services, Appeals Office, PO Box 64941, St. Paul, MN”
