Medication therapy management (MTM) is a service provided by Program HH through pharmacists. The program is designed to help you understand and take your HIV medications as they are prescribed. It is very important that you take your medication properly and not miss any doses.
A pharmacist providing medication therapy management will look at your medication history to ensure you do not have any drugs that may negatively interact. The pharmacist will also help you deal with any side effects you may experience as a result of taking your HIV medication.
Medication therapy management can help you by providing easy and ongoing access to a pharmacist who can answer your questions and assist you with side effects related to your HIV medication. The pharmacist can also provide you with medication adherence tools, such as pillboxes and other items to help remind you to take your medication.
Not all pharmacies are set up to provide medication therapy management services. Program HH Customer Care can assist you with locating a participating pharmacy. You can reach Program HH Customer Care at 651-431-2398.
