Long-term services and supports rates changes

Rate and budget changes on Jan. 1, 2026

DHS will implement the following rate and budget increases on Jan. 1, 2026.

Alternative Care (AC), Essential Community Supports (ECS) and Elderly Waiver (EW)

The following increases apply to AC, ECS and EW:

  • 6.013% increase to AC and EW monthly case mix budget caps.
  • 1.67% increase to ECS budget caps.
  • 6.013% increase to consumer directed community supports (CDCS) budgets under AC and EW.
  • 6.013% increase to EW customized living and 24-hour customized living daily and monthly limits. This change will happen on a rolling basis as lead agencies renew service plans throughout the year.
  • 6.34% increase to home-delivered meals.
  • 2.7% increase to environmental accessibility adaptations (EAA).

Disability waivers

The following changes apply to Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI) and Developmental Disabilities (DD) waivers.

  • 0.130% increase to CDCS under BI, CAC, CADI and DD.
  • Family residential services and life sharing services will move to a tiered rate methodology that will be implemented on a rolling basis throughout 2026.

We will publish the January 2026 frameworks in December 2025.

Home care nursing and home health services

The following increases apply to home care nursing and home health services:

  • 3.19% increase to home health aide, skilled nursing, occupational therapy, physical therapy, respiratory therapy and speech therapy (market basket adjustment).
  • 3.19% increase to home care nursing (market basket adjustment).
  • 3.19% increase to Consumer Support Grant (CSG) home care nursing ratings.

The same percentage increases also apply to extended home care services.

Personal care assistance (PCA), Community First Services and Supports (CFSS) and CSG

The following PCA/CFSS rate increases include changes to the wage component value and implementation factor within the PCA/CFSS rate framework:

  • 0.13% increase to 1:1 PCA/CFSS, PCA supervision and CFSS worker training and development.
  • 4.79% increase to PCA/CFSS complex.
  • 0.13% increase to CSG PCA ratings.

The same percentage increases also apply to extended PCA/CFSS.

MMIS automation process

The MMIS automation process is scheduled to run Dec. 10, 2025. DHS will provide additional information and instructions closer to that date.
