Ryan White services FAQ

If you are new to Ryan White services, download and complete the following forms:

• Ryan White New Application/Information Change
• Ryan White Annual Certification
• Ryan White Healthcare Provider Request (PDF)
• Ryan White Verification of Employer Insurance (PDF) (if you are employed). Include a copy (front and back) of your insurance card if available.

Also download and save a copy of the Ryan White Program Notice of Privacy Practices Rights and Responsibilities (PDF) to keep for your records.

• The Minnesota Council on HIV/AIDS hosts a complete list of HIV service providers in Minnesota. This list includes DHS-funded HIV service providers and Hennepin County-funded HIV services providers.
• You can find other HIV service providers across Minnesota. Although they are not funded by DHS, they might be funded by one of our county or state partners. You can find these services through partner websites, such as the Hennepin County Ryan White HIV Services or through an internet search.
• Additionally, the federal Health Resources & Services Administration hosts a map of Ryan White service providers across the country. This tool can be used to find the nearest HIV services to you.
