DHS manages publicly funded programs that support people with a variety of disabilities, including developmental disabilities, chronic medical conditions, acquired or traumatic brain injuries and physical disabilities.
Over the past two decades, Minnesota has successfully transitioned from high use of institutional services, such as regional treatment centers and nursing facilities, to effectively serve people with disabilities with flexible, cost-effective home and community-based services.
The Disability Hub MN is a free statewide service to help you find options to live independently. You can call 866-333-2466 Monday through Friday between 8:30 a.m. and 5 p.m. or visit the Disability Hub MN website for more information anytime. You can also use the live chat feature on the website to connect with a specialist during business hours.
