Four Steps to Success
Live Well at Home recommends a four-step method for achieving better, more independent living. When completed as a system, these steps form the needed foundation and structure that will help you flourish as you age. Here are the steps:
Step 1: Understand Your Resources
From caregiver support to your ability to complete daily tasks, take a snapshot of your daily resources – those you have and those you need.
Step 2: Make Your Choices
Decide which supports, services and products will be the most helpful to you in cultivating a life suited to your expectations.
Step 3: Take Action
With help from caregivers as needed, take the step of setting up the support services that you will use as you age.
Step 4: Look at Your Results
Ask yourself what’s working and what’s not and look for opportunities to update your choices when needed.
Planning for Care
There’s no formula for individual care needs – your mobility, activity levels and overall health will help determine care needs that are specific to you. One important consideration as you age is your access to long-term care - help with daily activities over a long period of time. Long-term care is a common support service for people over the age of 65, and it can be offered in your home, in your community or in special facilities. Start by understanding how long-term care can benefit you.
Find Services in Your Area
Your local Area Agency on Aging is a valuable resource for connecting you to the supports and services available in your area that you will keep you living well at home. There are seven AAA offices throughout Minnesota and each location is staffed with specialists who can help you ask the right questions and identify effective solutions to help you along the aging journey.
Get Involved in the Community
Beyond assessments, services and supports, the ideal path to successful aging at home includes forming a stronger connection to the larger community. Organized by the Minnesota Board on Aging, the Senior Linkage Line can serve as a resource to help you become more actively involved in your neighborhood, town or city.
