Gaps Analysis study request for proposal

The Minnesota Department of Human Services, through its Aging and Adult Services, Disability Services and Behavioral Health Divisions (State), is seeking Proposals from qualified Responders to develop and implement core processes for the State's home and community-based services system Gaps Analysis study.

Project outcomes - Working with the state, the successful responder will possess the appropriate professional and technical expertise to: 

• Assist with establishing and maintaining elements for connecting, supporting, as well as promoting and continuing to build efforts to address service capacity gaps at a regional and local level;
• Identify and develop and implement a communication plan and process that provides regular updates about, and share success from action planing activities, that respond to the regionally prioritized gaps in overall service capacity, and
• Arrange and convene a minimum of two meetings of Tribal Nations using the regional meeting framework developed in 2015-16.
