Consumer Support Grant Program

CSG is a state-funded alternative to the following Medical Assistance home care services:

The CSG program gives people greater flexibility in:

Purchase of goods and services.
Hiring support workers and serving as their employer.
Hiring their spouse or the parent of a minor.
Purchase nursing services from a home care agency (if eligible).

People may act as the employer of their direct support workers. For more information, see Participant Employer Options.

Community First Services and Supports (CFSS)

Beginning Oct. 1, 2024, DHS will transition people who use CSG and Personal care assistance (PCA) services to CFSS. After Oct. 1, when the county or tribal nation conducts a person's reassessment, they will give people who now use CSG information on how to transition to CFSS.

Eligibility

To be eligible for CSG, a person must:
- Meet eligibility requirements to receive HCN, HHA and/or PCA, which include:
  - Be eligible for Medical Assistance.
  - Be able to direct and purchase their own care and supports or have a family member, legal representative or other authorized representative do so on their behalf.
  - Have a functional limitation that requires ongoing supports to live in the community.
  - Live in their own home/family home and not in an institution.
- Not participate in:
- Have a PCA and/or HCN home care rating.
Covered services

A person may use CSG funds to purchase a variety of goods, supports and services beyond the usual home care categories. Covered goods and services must:
- Be directly related to the person’s functional limitations.
- Help delay or prevent out-of-home placement of the person.
- Be related to an assessed need.
- Not be covered by another source, such as Medical Assistance or private insurance.

Contact your local county agency or tribal nation to:
- Request an assessment to determine if you are eligible.
- Learn if your county or tribal nation administers CSG. If your county or tribal nation does not offer the Consumer Support Grant, they will reach out to a county or tribal nation that does to help you.
