How to become a certified BHH services provider

DHS must certify providers to deliver BHH services. DHS uses the term “organization” to refer to any medical clinic, community mental health center or other clinic or agency providing primary care or mental health services (or both). 

To obtain provider certification:

• An organization must be enrolled as a Medicaid provider and provide primary care or mental health services (or both).
• The organization must meet federal and state standards to become certified as a BHH services provider.
• All locations from which BHH services will be provided are enrolled in Minnesota Health Care Programs.
• All BHH services teams operated by the organization must meet the BHH services standards DHS-6766 (PDF).

Step 1: Familiarize yourself with the certification standards.

• Review Behavioral Health Home Services Standards DHS-6766 (PDF).

Step 2: Familiarize yourself with the interim certification process.

• Review the Behavioral Health Home (BHH) Services Policies and Procedures Certification Process and Application Guide DHS-3837 (PDF). 

Step 3: Request to apply.

• Complete and submit the Request to Apply for Behavioral Health Home Certification DHS-7093A.

Step 4: We will issue you a BHH ID and send a link to the behavioral health home services online application.

Step 5: Apply.

• Important: You must save the online application to your desktop or network drive as a PDF before entering information. 
• Close your browser and open the PDF you saved to your computer.
• Complete the PDF application and attach any required documents.
• Save your PDF.
• Click “submit” at the bottom of the application. (You must be connected to the internet before you submit your application.)
• DHS will review and process your application, and contact you with the next steps.
