AC provides home and community-based services to people who need nursing home level care but choose to live in the community. This program provides many of the same services as the Elderly Waiver program. AC is for people with low income and assets who are not eligible for Medical Assistance.
Family, friends and neighbors who provide care for an older adult or person with dementia can get many forms of support across the state. Caregiver consultants can meet with people one-on-one to provide support and guidance. Support groups, training, education and respite are also available.

MnCHOICES is an assessment and planning tool used by lead agencies. It uses a person-centered approach to help a person make decisions about their long-term services and supports.
PCA provides services to seniors and people with disabilities that help them remain independent in their community. PCA workers provide covered services in a person’s home or in the community.
Relocation service coordination is a type of case management to help people currently residing in eligible institutions who want to move into the community. RSC-TCM helps people plan and arrange for the services and supports they need to live in the community.
The Senior LinkAge Line is a free, statewide service of the Minnesota Board on Aging in partnership with regional organizations from around the state that focus on aging adults. The Senior LinkAge Line helps an older person and their caregivers find answers and connect with the services and supports they need.
The senior nutrition program provides dining sites and home-delivered meals in communities throughout Minnesota. Also, senior dining sites offer other social activities.
