Person-Centered, Informed Choice and Transition protocol
DHS created the Person-Centered, Informed Choice and Transition Protocol (PDF) as a guide lead agencies (counties, tribal organizations and managed care organizations) must use to implement person-centered practices. The protocol explains DHS expectations for lead agencies and others who do support planning for people who receive long-term supports and services, such as:
- Children and adults with disabilities
- Children and adults with mental illness
- People older than 65 years old.
Specifically, the protocol helps support planners use good practices and shows them how to develop and use person-centered plans.
