- Assistance related to your fee-for-service covered services
- Questions about eligibility for MinnesotaCare
- Reporting changes if you are enrolled in MinnesotaCare and no one in your household has Medical Assistance, including changes to the following:
- Your current address and phone number (if you do not tell us you have moved and returned mail has no forwarding address, coverage may end)
- Who lives with you
- Starting or stopping a job
- Other income that has started or stopped
- The people you plan to list as dependents on your income tax return
- Starting or stopping other health insurance
- Access to insurance through a job
- Whether you are pregnant or have a new baby
- Incarceration status
- Citizenship or immigration status
