Child care and early education: eDocs, forms and documents

These forms are used by providers to register with the county, tribe or subcontracted agency where the family receives assistance.

• Licensed Center Provider Registration and Acknowledgement DHS-5190 (PDF) is to register licensed child care center providers so that they are authorized to receive child care assistance payments.
• Licensed Family Child Care Provider Registration and Acknowledgement DHS-7195 (PDF) is used to register licensed family child care providers so they are authorized to receive child care assistance payments.

• Provider Registration Change Form DHS-7196 (PDF) is used by providers to report changes to their provider registration to agencies.

• Providers can submit the CCAP Quality Differential Rate Request Form DHS-4795 (PDF) to each county, tribe, and/or subcontracted agency where they are registered to show they qualify for the 15 percent quality differential by meeting accreditation or credential requirements. 
• Accrediting organizations can apply to have their accreditation approved by the Minnesota Department of Human Services.
