DHS is seeking to develop and implement a Pediatric Hospital to Home Pilot Program

DHS, through its Disability Services Division in collaboration with the Transition, Tribal and Transformation Division, is seeking to develop and implement a pilot program to facilitate pediatric hospital-to-home discharges for patients receiving Minnesota medical assistance services.

The term of any resulting contract is anticipated to be from July 1, 2025 until June 30, 2027; however, most of the pilot program must be completed by June 30, 2026. See section 2.2 of this RFP for additional details.
