Notice about recent data breach involving some DHS clients and potential clients

On July 2, 2024, a Minnesota Department of Human Services (DHS) employee received a phishing email that compromised the employee’s state-issued email account. An internal review of the employee’s email account identified 4,329 individuals whose data could have been accessed. DHS is providing notice to individuals whose information was potentially accessed, informing them of the incident and what they can do protect themselves. We value the privacy of the people we serve and will continue to work with our employees and partners to prevent data privacy incidents. Agency transition overview

On May 23, 2023, Gov. Tim Walz signed into law two pieces of legislation that will bring big changes to the Department of Human Services, paving the way for the creation of a new Department of Children, Youth, and Families (DCYF) by July 1, 2024, and establishing the Department of Direct Care and Treatment (DCT) as a standalone agency in 2025.

DHS will mail the form only when requested by Medical Assistance and MinnesotaCare enrollees. We will not automatically send the form out. Follow the instructions on this webpage to request your form 1095-B.
