On July 4, 2025, President Trump signed new law that brings sweeping changes to Medicaid, a federal health insurance program that is called Medical Assistance in Minnesota. All states must implement the changes in the law, which include additional requirements for eligibility.

In keeping with our mission to help Minnesotans live their healthiest and fullest lives, the Minnesota Department of Human Services will keep you informed about the coming changes so you can plan for yourself and your family if you are directly affected by changes. 

Nothing has changed yet. The first changes won’t take effect until fall 2026. This gives DHS time to review the details, get more information from the federal government, and communicate the changes clearly to Minnesotans. Check this webpage for the latest updates about how these changes may affect you. We will add content as we get more information. 


What are the changes, who is affected and when?

What: Work requirements
Who: People ages 21 to 64 who do not have dependent children, are not pregnant and are not receiving Medical Assistance based on a disability
When: Jan. 1, 2027 

What: More frequent renewals
Who: People ages 21 to 64 who do not have dependent children, are not pregnant and are not receiving Medical Assistance based on a disability
When: Jan. 1, 2027 

What: Limits on retroactive coverage
Who: People who apply for Medical Assistance on or after Jan. 1, 2027
When: Jan. 1, 2027 

What: Cost sharing on some services
Who: Some people ages 21 to 64 who do not have dependent children, are not pregnant and are not receiving Medical Assistance based on disability
When: Oct. 1, 2028 

What: Change in eligibility for some immigrants
Who: Only certain lawful permanent residents, Cuban and Haitian entrants, and Compact of Free Association (COFA) migrants will be eligible. People who would not qualify include adults over 21 who are refugees, humanitarian parolees, asylum grantees, certain abused spouses and children and victims of human trafficking, and others granted protection under a humanitarian basis.

What should I do?

There is nothing different you need to do now to keep your health insurance. Remember to:  

  • Make sure we have your current contact information.  
  • Watch your mail for an envelope with a blue circle when it’s time to renew and complete your renewal right away. 
  • Visit mnrenewallookup.com for details about your coverage. 
  • Check this webpage and watch for other communications from DHS.

Who will not be affected?

Medical Assistance plays a major role in contributing to Minnesota’s health care sector and public health, so many Minnesotans will likely experience some downstream effects from these significant changes. However most current Medical Assistance enrollees will not be directly impacted by these changes if they:

  • Get coverage based on a disability 
  • Are 65 or older 
  • Are 20 or younger 
  • Are pregnant 

What’s changing for private health plans?

Federal changes could mean less financial help for some families. But many Minnesotans will still qualify for big savings on private health plans through MNsure.
