You received your renewal information in the mail.
Here’s what’s next:
- Open it.
- Review and complete it.
- Return it as soon as possible to avoid delays in coverage.
If you are already enrolled in other health insurance and no longer need your Medical Assistance or MinnesotaCare, call your state, county or Tribal agency and let them know.
Open your renewal envelope, review and complete the forms
What you need to do next depends on what type of form or notice you receive. When you open your renewal envelope, you’ll either see:

Read instructions about how to complete the form and how to get help with it if you need it. Make sure your information is complete and accurate.

Notice that your insurance automatically renewed
Make sure your information printed on the notice is correct.
- If it’s correct, you’re done.
- If not, write in your corrections and submit the document.

Submit your completed form
It's important that you submit your completed renewal form to your county, Tribal or state agency as quickly as possible to avoid gaps in coverage.
You can submit your forms in any of these ways:
Online
Upload the documents online using the Upload documents button.
Upload documentsMail, fax or in person
Mail: Put documents in the return envelope provided, add postage and put it in the mail. If you don’t have the return envelope, you can use your own and send it to your local county or Tribal office.
Fax or in person: Drop off the documents or fax them to the agency listed as the return address on your paperwork.
