People with breast or cervical cancer

This program is for younger uninsured Minnesotans who have been identified through the SAGE program screening as needing treatment for Breast and Cervical Cancer. This includes people who meet all the following:

  • Are screened through the Minnesota Department of Health Sage Screening Program or another CDC-funded cancer screening program
  • Diagnosed as needing treatment for breast cancer, cervical cancer or a condition that could lead to cervical cancer
  • Do not have health insurance that covers treatment
  • Are ages 30-64
  • Live in Minnesota
  • Are a U.S. citizen or are a qualified noncitizen

Ways you can apply

You can choose one of these ways to apply depending on what is easiest for you and your family.

Mail, fax or in person

For one year of coverage, complete the Application for Breast and Cervical Cancer Coverage and send it to your local service agency by mail or fax or drop it off in-person.

Prepare to apply

When you apply breast or cervical cancer coverage, you’ll need to provide some information about you and your household and some additional items.

Gather documents

Use the list below to help you gather what you may need to apply. Your worker will attempt to verify this information using our data sources, but if they are unable to verify any of these items, paper proof of the following may be required:

  • Screening for treatment through the Minnesota Department of Health Sage Screening Program.
    Note: Sage screening should be done BEFORE you apply
  • You are not enrolled in other health care coverage
  • You live in Minnesota
  • You are a US citizen or qualified noncitizen 
  • Your Social Security Number
  • Verifying income is not mandatory, but there are income limits to be found eligible. (250% of federal poverty limits)

Who to include

The following people should be included in your household when applying for coverage:

  • People who need treatment of breast or cervical cancer
  • Any authorized representative acting on their behalf

Who NOT to include

The following people should not be considered in your household when applying for coverage:

  • Anyone else in your household who doesn’t need treatment of breast or cervical cancer
  • Anyone over age 65
  • Anyone ages 21-29
  • Anyone with existing health care coverage
  • Anyone who does not require treatment for breast or cervical cancer
  • Anyone who has NOT been screened by the SAGE program
