    Therapeutic area - Endometriosis

    Approval criteria

    • Patient must have a diagnosis of endometriosis AND
    • Provider must provide a very compelling reason as to why Lupaneta Pack is required

    Therapy limitations

    Initial course of treatment will be limited to 6 months with maximum therapy limited to 1 year, due to the potential of adverse impact on bone density.
