Rationale

• Procrit and Epogen are both manufactured by Amgen
• Procrit is distributed by Ortho Biotech
• Epogen is distributed by Amgen
• Both products are available in the same strengths and package sizes
• Both products share the same warnings and precautions
• Both products are dosed as Units per Kg

Criteria

A PA may be provided temporarily during the period of Epogen and Procrit recalls effective November 3, 2010.

Established criteria

Patient is self-administering and because of mental health disabilities, cannot be switched to the preferred epoetin alpha, Procrit.
