Special Needs BasicCare directory

Special Needs BasicCare (SNBC) is a voluntary managed care program for people with certified disabilities who are 18-64 years old and have Medical Assistance. There are two different SNBC plan types available for people to choose from, SNBC Non-integrated health plans (SNBC-NI) and Integrated SNBC health plans (I-SNBC)

SNBC is available through different health plans depending on the county you live in. You can find which health plans are available to you by searching the directory below, or looking at the SNBC health plan map.

Call the health plan directly if you:

  • Already have Medical Assistance (MA) and you want to enroll in SNBC.
  • Have questions about covered services.

SNBC Non-integrated plans (SNBC-NI)

People choosing to enroll in this option get their Medical Assistance health services covered by the health plan. Waiver and PCA/CFSS services are covered by fee-for-service Medical Assistance. Contact Disability Hub MN to enroll or disenroll.

Disability Hub MN

Integrated SNBC plans (I-SNBC)

People enrolled in this option get both their Medical Assistance and Medicare services (including Part D) combined into one plan. To join I-SNBC, a person needs to already have Medicare. Waiver and PCA/CFSS services are covered by fee-for-service Medical Assistance. Contact the health plan directly to enroll or disenroll.
