Apply for health care
People who need family planning services
Each person applying for Minnesota Family Planning Program (MFPP) will need their own application, unlike other programs.
For those who only need help paying for family planning services, related supplies and transportation to and from the providers of the service. This includes people who meet all the following:
- Are living in Minnesota
- Are people between the ages of 15 and 50
- Are not currently pregnant
- Are US citizens or considered a qualified non-citizen
- Are NOT already enrolled in Medical Assistance or MinnesotaCare health care programs
- Are under 200% of the federal poverty income level
Note: For those under 21, parents’ income does not count towards their income

For one year of coverage, complete the Minnesota Family Planning Program Application and Renewal Form and return it by mail to:

We will look at your application to determine whether you can get family planning health care coverage for one year.
- We will make this decision within 45 days.
- If we need more information, we will call you or send you a letter in the mail.

Can I get health care services right away?
You can apply for short-term family planning coverage at certain clinics. To get the names of these clinics, use the directory button below and select "Family Planning Services from the "Type" drop down and "Certified MFPP Provider" from the "Sub Type" drop down. You can also call 651-431-3480 (Twin Cities metro area) or 888-702-9968 (outside the Twin Cities metro area).
- Short-term coverage starts right away.
- Short-term coverage ends on the day eligibility for ongoing family planning services is determined or, if no application is submitted, on the last day of the next month.
- You can get short-term coverage only once a year.
- To get family planning services for a full year, you must send an application to the Minnesota Family Planning Program.
- The clinic that gives you short-term coverage can help you apply.
- The Minnesota Family Planning Program may also pay for family planning services going back three months before the month of application.
Prepare to apply
When you apply for Family Planning Services, you’ll need to provide some information about you and your household, including income, and some additional items. Use the list below to help you gather what you may need to apply.
Gather documents
Your worker will attempt to verify this information using our data sources, but if they are unable to verify any of these items, paper proof may be required:
- Social Security number for each person applying
- Date of birth for everyone in household (not just those applying)
- Driver's license, Tribal ID or other ID
- For non-citizens, Green Card or other immigration documents
- Documents for sources of income for each person applying
Who to include
The following people should be included in your household when applying for coverage:
Note: Even if someone doesn’t need health coverage, they still include them if they fall into one of these groups.
- You
- Your husband or wife
- Your children under 21 who live with you
- Your spouse’s children under 21 who live with you
- Any unborn children you or your spouse are carrying
Who NOT to include
The following people should not be considered in your household when applying for coverage:
- If you are under 18, you do not need to include your parents
- Anyone else living with you did not list in the Who to include section
