MnCHOICES
MnCHOICES is a computer application used by counties, tribal nations and managed care organizations to support their assessment and support planning work for Minnesotans who need long-term services and supports (LTSS), regardless of age, type of disability or service needs.
Certified assessors, care coordinators and case managers work with people of any age with a disability or anyone in need of long-term services and supports. They use MnCHOICES to discuss your preferences and the supports you need to live a good life. Your responses determine eligibility for a number of public programs, including home and community-based services waivers and Community First Services and Supports. Its rules-based system ensures that everyone is treated equitably.
To improve the state's long-term services and supports assessment and support planning process, the department worked with FEI Systems to develop the MnCHOICES computer application. The application:
- Eliminates the need for multiple assessments to receive needed supports and services.
- Helps you choose how you want to live, learn, work and play through the assessment and support-planning process.
- Ensures consistency and equal access to services and supports statewide.
MnCHOICES assessments
Certified assessors uses the MnCHOICES assessment to determine program and service eligibility for people. A certified assessor meets with a person to gather information about their needs. After the assessment is complete, the certified assessor reviews a summary of the assessment and provides referrals, resources and next steps for the person.

If you are enrolled in a managed care plan, your care coordinator will contact you to schedule a health risk assessment (HRA). The HRA helps you identify and understand your health risks and monitors your health status over time. The HRA tells you what you can do to reduce risks, stay healthy and prevent disease. An HRA cannot be used to determine eligibility for waiver programs, Community First Services and Supports or Housing Stabilization Service. You may also receive a MnCHOICES assessment to see if you are eligible for these services.
If you are enrolled in a managed care plan, a care coordinator will work with you to:
- Understand your health care needs, health status and risk factors.
- Find out what kind of help you need to support you in your daily activities, such as bathing, grocery shopping, transportation, etc.
When you and your care coordinator complete a health risk assessment, you will receive a support plan that provides you with important information about what type of support you will receive, how often you need it and when you should contact your care coordinator.
Support plans
When you complete a MnCHOICES assessment or health risk assessment, and if you qualify and choose to receive publicly funded services, a staff member from your county/tribal nation or managed-care organization will work with you to create a person-centered support plan .
- Support plans after a MnCHOICES assessment: You will work with a staff member to identify your needs and support options according to your cultural choices, preferences, interests and goals. The support plan outlines the decisions you make about the services and supports for which you are eligible. You will receive a printed support plan that includes information on each of the services you chose, your provider’s contact information, your staff member’s contact information and other planning information.
- Support plans after a health risk assessment (HRA): You and a staff member will use the information from your HRA to develop a support plan. The HRA support plan outlines the decisions you make to meet your health care goals. The plan may include referrals, the type of services to be provided, frequency of support from the care coordinator and when and how to contact the care coordinator.
