Completed renewal forms can be submitted in one of several ways to your county, Tribal or state agency.
Mail, fax or in person
Mail: Put documents in the return envelope provided and send it to the agency listed as the return address on your paperwork.
Fax or in person: Drop off the documents or fax them to the agency listed as the return address on your paperwork.
