Communities Coordinating for Healthy Development (CCHD)

Communities Coordinating for Healthy Development (CCHD) is a quality improvement project designed to improve child outcomes through increased communication, care coordination, and collaboration between the child’s primary care clinic, early intervention/preschool special education providers, families, and other community partners. 

The project’s purpose is to create a referral feedback loop for developmental and socio/emotional health screening for children 0-5 years old. This toolkit and training materials are designed to help clinical staff, early intervention and preschool special education providers, and other community stakeholders successfully implement a CCHD project. 

This toolkit and training materials are still in development and being reviewed by pilot teams that participated in the original ABCD III project. This message will be removed when the materials are finalized.
