Approval criteria
- Patient must be at least 18 years old AND
- Patient must have a diagnosis of nasal polyps AND
- Xhance is prescribed by, or in consultation with, an ENT specialist AND
- Patient must have an adherent and adequate trial and failure of at least 2 different preferred intranasal corticosteroids AND
- Patient attests that instructions for self-administration of Xhance have been provided and understood
